In addition to such other information deemed necessary by the license officer, the application shall require the disclosure of the number of employees at each location and state whether the owner, or owners, work in the business at each specific location. If the applicant is claiming an exemption or deduction under the terms of the chapter, the application, in addition, shall include a full disclosure of those facts pertinent to the exemption or deduction.
